New managers need to apply for Risk Management Cards ASAP.

Five Forms necessary for player registration:

MYSL or MSPSP Registration form       MSYSA Medical Release (Notarized)

MSYSA Permission to Travel                 Club Waiver

Club Parent/Player Contract

Birth Certificates necessary only for any new players-once a player has been issued a pass card, proof of birth will not be needed for subsequent seasons.

Managers please share player payment plan with parents so they know player fees and when payments are due.
